I removed the fuel pump to replace it and also removed the fuel accumulator. When I removed the fuel accumulator, fuel also came out of the small curved vent at the back. I am assuming that the accumulator is also bad since fuel should only be in the end with the fittings. Is this correct?

Fuel should not exit via the vent tube on the bottom.

I believe that yes, fuel leaking woud indicate that the diaphragm is leaking and allowing both pressure loss (hard warm start) and fuel to leak from that little tube. Only fix is to replace.

    See: http://www.ferrarichat.com/forum/mondial/424171-fuel-accumulator.html
    for a brief confirmation.

One of the PO's of my car connected the vent tube back to the fuel tank breather! I believe it's there as a tell-tale for a faulty diaphragm

I believe all USA cars had that tube routed back into the tank. TAV11 on the 308QV USA clearly shows it. I don't know if ROW cars had that or not, but the Euro 308QV diagram on Ricambi's site does not show it.
