    why the diff failed...

The side gear rings incorporate the "ramps" which determine how quickly the diff locks up, you want slower lock up under power but quick on the overrun, so a wheel doesn't lock when braking for a corner. The side gear rings come as matched pairs, the ramp has to match so that the diff gears apply the same load at the same time. See first picture for the new matched set.

Some doofus mechanic in the past has had a side gear break, but rather than order a new pair, he's ground down one out of a completely different diff. The result is the ramps are completely mismatched. The load was much higher on the crownwheel side, thus a drive dog broke off.
    Second picture shows the mismatch.

My guess would be it was done in the pits and he worked with what he could get?

that could indeed be true, but I'm more cynical than you - the side gear has been precision surface ground, the sort of thing that needs a machine shop, not a pit garage. I'd say it was repaired as cheaply as possible before the car went on the market. A bit like buying 90% of used Ferraris - you get all the previous owners' short cuts.

Not really, PP is one of a very short list of people who I would buy a used Ferrari off without hesitation.

What happens after PP sells them is another matter...
