Looks like only the the tailpipes and or the mufflers which makes sense since the 250 GTO had the dual layer pipes. The original buyer was probably looking to recreate that look (I was always a bit surprised why Ferrari used that look for the HGTE but not for the GTO).

Affirmative, many owners love the Snap style twin wall tailpipes fitted to various HGTC/HGTS/HGTE Ferrari models. About the only reason I can think of. My 575M has HGTC mufflers and tailpipes and I really like the look. Plus, they stay cleaner.
"Not possible" per atelier, because the original blue fabric was not compliant w/contemporary standards of 599 GTO era.

A decade or so ago, we were fortune enough to purchase the ex-Niki Lauda 288 GTO, because it lived at Ferrari's Maranello Museo on display, that deal introduced me to a number of Ferrari SpA insiders especially it's renowned project Chief Franco Cimatti (imaged below) who's always been a fan of the GTO. Over the years he's been helpful with factory information where I've needed help or clarification on something. Recently, he pointed out that the 599 GTO was Stefano Carmassi's baby, as he said to me: "At the time, 2008-2010, he was in my concepts development team. I assigned him to the combined 599XX - 599 GTO projects. He oversaw the complete work from time zero to final delivery." Good to know because Carmassi is credited with key roles in everything special from the Enzo to the current Le Mans winning 499P. Yes please. A late friend of mine, a long term F40 owner drove a GTO over a mountain passes and described the car having a similar "thrill factor" to his F40.
You mean manual gearbox? As far as I understand it the 599 GTO's transmission's focus was track-focused, so the F1 six-speed gearbox not only uses SF2 software to provide the much-talked-about 60-millisecond shift times from the single-clutch automated manual with a manettino setting of race or higher, it's party trick is multiple downshifts by engaging the left carbon-fiber paddle shifter, just as on the 599XX. Therefore a manual gearbox is redundant in this car. If you think of the driver-car interface with Virtual Race Engineer (VRE) which provides instant performance information, it's so far removed from a manual gearbox application.

As assigned by Franco Cimatti, the 599 GTO team comprised Stefano Carmassi who oversaw the entire 599XX & 599 GTO projects from start to finish, he appointed Fernandino Canizzo to manage the car's bodywork & aero, award-winning powertrain chief engineer Vittorio Dini to develop the GTO's engine, and Martino Cavanna who oversaw the GTO's testing & development. Unfortunately not in English (which is puzzling as Ferrari SpA's Vittorio Dini speaks perfect English!) he explains the 599 GTO's potent blueprinted engine also from 2010. If you understand Italian it's a fascinating piece and even if you don't you get the idea by watching the video.
