Revving an engine like that with no load on it is so bad for the engine. Are the people aware of that and don't they care or is it news to them that this is horrible for any engine?

Erm, it's worse for an engine when it is loaded, hence heel and toe + double declutching. A loaded engine ie driveline engaged causes greater stress on all of it's components.
