Name your own blooper, a faux pas you had witnessed.... Here is mine, took place at work (software company). I was working with engineers in India. When they heard that translation was expensive (for words on the screen) they decided to abbreviate a whole bunch of stuff and put acronyms in other places. One of the cases was an icon with a label of "Terms And Agreements". The new mockup I reviewed a couple of weeks later had the unfortunate abbreviation of "T&A" !!!! (Think American FratBoy speak).
