Not sure about the 3.2, but on the QV (as per the owner's manual), cold start is without touching the pedal. If you open the throttle when cold, it gets too much air, the mixture is too lean and it won't start properly. When hot, it says that you may open the throttle a little during start-up.
    What does your manual say?

Yes - the CSV is disconnected ;) Seriously, you probably have a couple of problems going on:

Proper cold-starting requires the CSV + TTS system to work. Measure the voltage between the two wires in the CSV connector when unplugged during the first cold starter motor cranking attempt. It should go +12V for a second, or two, and then go to 0V. If it always stays +12V = probably the TTS has failed closed, and then a correct temporary fix would be to unplug it - but that's no long-term solution. However, people often (wrongly) unplug the CSV when having warm-restart trouble -- and it can be the trouble if something is truly wrong with the CSV+TTS system, but that's not the typical cause of warm-restart trouble.

Warm-restarting requires the fuel system to hold pressure after engine shut-off and that requires three things:

    1. a non-leaking accumulator (since yours is new = not this),
    2. a non-leaking check valve at the fuel pump outlet (= top suspect), and
    3. a non-leaking check valve in the pressure regulator section of the K-Jet fuel distributor (a lesser suspect)

Have a Bosch CIS specialist (a vintage Volvo/Mercedes shop), or yourself, measure the fuel pressure after engine shutoff - if it immediately falls to zero = he/she/you needs to fix 2, or 3.

    Good Hunting!

Have yet to change them. Do you have other PN (maybe Bosch?)?

I tried cold start a few times without touching the pedal, for more than 10sec each time but failed to start the engine :(

I'll try to measure the CSV voltage this weekend. My top suspect is loss of fuel pressure due to bad check valve instead of electronics as I can re-start the car without any problem within 30sec after switching off the engine.

Many thanks guys for all your help!!

Is your engine running very lean?

The CSV only operates at the following coolant temperatures:
