If I've got a full tank of fuel, and gauge is on empty and the low fuel warning light is on, has the sender unit failed on my 22k mile 11 year old 456GT ?

Is this a big job to fix ? and how do you go about it - under car, of from car interior or boot ?

Not a big job. Go through the trunk. Siphon out the tank. Sender is a float type and readily available. You will need a large spanner wrench to loosen the panel that the float is connected too.
