just found this thread and found it interesting, in that in the 7 years and 20,000+ miles I've put on my 328 I've suffered the same problem. Luckily, I've discovered the problem and solved it both times.
Had severe head aches even after just a few miles. The car had a major service and the problem still existed. Smog checks OK. No visible leaks anywhere. No changes in fluid levels. First hint was a film that kept showing up on my windows, which took about 2-3 months to show up. Put the car up on jacks, removed the tires, inner wheel wells and totally cleaned the engine by hand with berrymens chemtool. When I was done there was no road dirt or anything else on the engine, transmission or engine bay. Put the car back together and drove it . The head aches continued.
A month later the car went back up on the blocks and was taken apart again. This time I got out a bright light and started looking over every inch of the engine, transmission and engine bay. What I found was a little black spot on the rear header shield just under the A/C compressor. Since I work in the A/C field I looked at the compressor and discovered that the shaft seal on the compressor was leaking just a little bit of oil, dripping on the heat shield burning up. That little bit of burned oil would make it's way back into the car and cause the head aches. Fixed the problem and no more head aches. Car was good for about 3 years and I started getting head aches again.
This time I checked the A/C but was still fine. Figuring it was oil hitting the exhaust again I narrowed my search. This time I found the passenger side half shaft seal was flinging small amounts of oil onto the underside of the rear exhaust cover. Pulled the shaft and replaced the seal. Again the head aches went away. That was a year ago and still no problems.
Hope this helps someone, those headaches almost made me sell the car.

I get them once in awhile. Most recently when I was giving rides to a friend of mine and his family. Windows down and in a CS. Had a good headache when I got home. I attribute mine to dehydration and noise. My headaches arent common but do happen from time to time.

I agree that's the most likely. I got the occasional headache in the 328 because it's a loud car, maybe moreso with the Tubi it had.

IIRC one of the reasons almost no V8s are made with flat-plane crankshafts is that the resonance/noise is viewed as a drawback.

Definitely check for gas/fume issues, but I would try the earplug experiment (if legal) and see if that resolves it.

I get headaches when viewing the driving of idiots and the clueless.
If I drive at night or on clear roads they go away.

Problem is either You or the Car. Noticed that you went over a year with no headache problems. If so, unless something changed about the car, or you did no driving during that time, that narrows the problem down to you. There are other clues in this thread that it may not be the car. You had a drive with the windows up and did fine. That rules out CO, road vibration, seating position, etc. Air flow resonance, when the Windows are open could still be on the list. Still, no symptoms for a year? What changed? You answer that question and you'll figure out the problem. Do you get headaches flying the L-39? Older jets have many of the same environmental stressors as older cars. Fumes, vibration, ergonomics, are similar or worse. Next time you see your flight surgeon, talk to him about it too. Hydration is huge. Hydrate well, like your going to go fly, then drive and see what happens. People here have mostly given you serious and excellent suggestions. Maybe start eliminating each possibility one at a time. Best of luck!

I've experienced this a few times in some vehicles and I believe it was the A/C. I'm not sure if you drove with or without the a/c during these times but it gave me a few problems.

I usually drive my 308 when I do have a headache, it goes away as soon as I hit fourth gear! Ferrari for me is a stress releiver!

All three drives with the top on right? If so try the ear plug suggestion with the top off as you can still hear everything yet in a more subtle less offensive tone/volume to your ears.

Hope you figure it out soon.

    Regards,
    Gregg

ps, I am glad a ran across this topic as i am volunteering this weekend at MSR and would have forgotten ear protection.

No headaches with the 360. I think the 348 had more of a gas smell.
Glad I don't get headaches anymore.
