A Lufthansa Airbus A340-600, registration D-AIHM performing flight LH-411 from New York JFK,NY (USA) to Munich (Germany), was cleared for takeoff from JFK's runway 22R at approximately 18:52L (22:52Z), the next aircraft in sequence had been cleared to line up runway 22R, when the tower controller highy agitated called "CANCEL TAKEOFF CLEARANCE" at approximately 18:53L, Lufthansa 411 reported rejecting takeoff. The airplane slowed safely from high speed. Another crew on frequency commented that was very close. The Airbus A340 vacated the runway at taxiway H. The crew reported hot brakes and had the brakes checked out by emergency services.

At the same time an Egyptair Boeing 777-300, registration SU-GDR performing flight MS-986 from New York JFK,NY (USA) to Cairo (Egypt), was in contact with ground control taxiing for departure out of terminal 4 and had been instructed to turn left onto taxiway B hold short of D. The crew however missed the turn onto taxiway B and instead turned onto runway 22R at taxiway J. Subsequently ground control calmly instructed the aircraft to turn back onto J and turn right onto B, then changed plan and had the airplane taxi up runway 04R turn onto taxiway H and join taxiway B.

The Lufthansa A340 was able to depart again about 2 hours later and reached Munich with a delay of 1:45 hours. The Egyptair B777 departed New York about 75 minutes later and reached Cairo with a delay of 40 minutes.

Very scarey. Third world airline crew nearly kills 400+ people. Great save by the tower, although I am sure the Lufthansa crew saw the 777 about the time they aborted. Whether that would have been enough time to stop without the warning is another question.

As I have not been in a pilot's seat in over 20 years ago (and then, it was for lessons in a single engine, which I didn't pursue due to other commitments), I ask this with almost total ignorance of how airports currently "fail safe" a runway from something like this. If the problem was that the Egyptair 777 on the taxiway missed the turn off, presumably, there are other highly-noticeable signals that should indicate to them that they're about to enter onto an active runway. Aren't there? I mean, if there aren't, that ought to be easily addressed. Paint the taxiways red where they border an active runway. Have red flashing lights at the active runway's edge. It's possible to have too much going on, of course, and still blow through everything right out into the path of an oncoming plane.

I have to admit that looking at that airport map, it's dizzying. And, unless you're familiar with the layout of the airport, I could see how mistakes get made. The left seat should be reserved for someone who knows the lay of the land, right? But, things do come up that distract you at the worse possible times.

From what I understand now, the EgyptAir aircraft did not actually make it to the runway, but did blow through the hold line, and that is what made the tower controller go appropriately ballistic.

Taxiing around airports is one of the most confusing things a pilot faces, many airports now have all sorts of measures to help prevent that. Sensors at hold short lines set off an alarm in the tower is someones nose crosses it. Boston and Orlando, I noticed lately, have red lights in the runway or taxiway that come on when another airplane is crossing that runway or an airplane is taking off from that runway, Also several hold short lines have bright flashing yellow lights. Larger airports have airport surface radar that plots every airplane taxiing with a transponder on on a large screen.
