    Does anyone know if/where there is an air OUTLET for the interior?

    Correct, for the exterior side. But where in the interior would the air pass through to leave the vehicle via that outside vent?
    The only passage I can figure out is via the slots where the seatbelts pass through the side trim.
    That seems quite insufficient in terms of section and would explain why the fan is struggling to create any flow on A/E position. (external intake flap open)
    On A/C, where it re-circulates the air directly from below the IP and the external flap is shut, the fan airflow is almost acceptable as it does not have to fight against the pressure inside the cabin.
    Next time I take that side trim off, I'll check this out and will post my "findings".
