Quote from NTSB seems a little sensationalist, re 5 mi separation. If landing on parallel runways the separation is whatever the runway separation is, which in the case of Sky Harbor is about 3500ft (<<1mi). From the tracks shown in the link it almost appears as though Delta overshot their turn to final. I'm sure there will be better analysis posted online soon.
