Need help finding a gerontologist or other PCP upper East Side

I'm a Radiologist and grew up in New York but have zero contacts there. My stepmom is 90 & lives on the Upper East Side on 64th b/w York and First. Yes, I know what's nearby just don't know anybody. Well, my brother (he's in California, me Texas) and son (Texas) have noticed the phone call repetitions have now become worrisome. We're gonna coordinate schedules for visits this year. ('ol battle axe ain't movin out!!!!!!!!) We've already arranged to have food delivered and will be working on "non health/welfare check" visiting agencies over the next few weeks. I know she had become disillusioned with her previous PCP and hadn't gone in a few years so I've been tasked with trying to find a gerontologist/PCP for her, ideally one who can speak Italian- not due to language rather to ease the transition. She's fluent and worked in the fashion industry.

Any help would be greatly appreciated.
