Robin- Possible. The 599 gearbox case (with 612 internals) and twin disc clutch might complicate things since there would be a combination of 599 and 612 parts needed for the conversion. Likely why Ferrari never bothered. That and no demand.
There would be very few parts different on a manual OTO and a regular manual 612. The clutch slave cylinder is different as you say, but it’s a simple process of changing the seal material to EPDM (brake fluid compatible), which is exactly what Ferrari did on the manual 599’s. It’s arguably simpler than converting a non-OTO 612 which has a completely different slave cylinder from manual to f1, which leads onto needing a different bell housing and torque tube if you want to use original parts. There was demand. They had my order in hand. They didn't do it because they didn't want to certify two separate drivetrains for the US market, and that's what it would have taken.

I was rear ended at lights in central London a few years back. Waiting there, saw in my mirror a woman in a 4x4 chatting on her phone and approaching fast. Put my foot on the brake in order not to be pushed into traffic and Bam. She got out and starting yelling at me for reversing into her! And it was only because of the car I was in that I had my story backed up, because a guy had been standing by the side of the road videoing my car, which was..a brand new Ferrari 612, on loan from Ferrari. He came up, gave me his details, and she went silent. Her car had bangs and scrapes all over.
It’s a sad day in the world when you have to consider buying a traffic camera just to protect. I was involved in a head on earlier this year but was able to turn away at the second. Thankfully, the guy stopped and admitted guilt. I’m seriously considering one of those M1’s from escort it’s so bad nowadays but don’t want to lose window real estate
