I don't think there is really any P&R content.... Apart from the people dying it's kinda a funny story and also a good reminder that you can never take safety for granted. Some of the closest calls I have had while flying have been with high time senior officers.

Seems like they are making the right move in all this by bringing in a outside investigative body and my assumption is that they are closest to us regulation wise. We don't even have to file an 1801 (DoD International Flight plan) when flying from the US to Canada in many circumstances.

Just was telling my wife about this and she saw it on CNN yesterday so it has made the news, it's just a minor event.

It's sad. Mid-airs are tough... while theoretically they should have been "seeing and avoiding" it's simply not always possible. Once in a while, the odds catch up to you.

Very sad accident. Hits a little too close to home for me.

The linked news article identifies the 3 people involved:

"In one plane was Thomas Proven, 70, a crash investigator with the FAA, which regulates all air travel in the U.S.
In the other were pilot Mike Duncan, 60, chief medical officer with the NTSB - the lead U.S. air-crash investigation body - and passenger Paul Gardella, 57, a veteran flight instructor with Virginia-based Aviation Adventures."

Aviation Adventures is one of two flight schools at my home airport JYO (Leesburg, VA) and has a good reputation.
Apparently Paul Gardella worked for Aviation Adventures in their Warrenton, VA location.

While the incident itself is sad, I appreciate of the professionalism of the FAA and NTSB. It isn't at all hard to imagine either of the bodies keeping the matter 'in house' rather than doing what they did: asking for impartial outside assessors.

I'll be at Paul's memorial this weekend, he was a friend. On top of being a very careful and diligent instructor he was also one of the nicest guys I have met in aviation.
