Just an aside - Keith Urban is a nice bloke: his brother is in my surf club and came home to find a new 4WD in his driveway this week!

It's widely reported in the US that obesity is the primary co-morbidity and obesity is disproportionally common amongst lower income populations and, dare I say it, black and latino people. Obesity also brings a massive jump in heart disease and diabetes for people above middle age.

Melbourne has higher density populations of low income people nearer to the city than Sydney. Could this be a factor?
