    We blew up the Modular Insertion Stage when a defective pressure gauge allowed an overpressure of a tank. Those high-pressure tanks can be bombs. We lucked out that the resulting explosion only broke the arm of a tech who was working nearby. Completely destroyed our upper stage and another stage that was in the same facility in Arizona.
